Recommendation.

  1. Download and install the latest Adobe Reader program to your computer.

    We cannot guarantee that older versions of Adobe Reader (prior to version 9) will allow users to open, complete, and save data in our PDF forms. Nor can we gaurantee that the forms will work properly with other PDF readers and editors, such as Foxlt Reader or CutePDF.


  2. Download and save the form to your computer before entering any data
    In most internet browsers (Internet Explorer, Chrome, Firefox, Safari), a new window will open and your internet browser will display the form in their own PDF viewer or reader.

  3. You will need to change your particular browser settings for Adobe Plug-ins, such as disabling Chrome's PDF Viewer.

    This should only need to be done one time.

  4. Click the form number. You will be prompted to select either "Save as" or "Same target as" from a pop-up box.

  5. Assign a name to the form and choose an appropriate location on your computer to save the file.

  6. Save archival copies of completed forms to separate client folders.
